If you would like to receive a RT-PCR test for COVID-19 in our hospital, please understand the following information.

◆Eligibility for issuing health certificates

・Issuing health certificates is limited only for the cases that border control requires advance submission of the medical certificate. Information should be confirmable on the official website or by documents of official bodies such as governments.

・Please directly confirm the requirements needed for your application, such as examination items, designated forms, etc. and conditions such as the period from diagnosis to submission with embassies before making an appointment.

◆How to Make an Appointment

Before visiting our hospital, please make an appointment via telephone by following phone number on weekdays (8:30-17:00). As our general rule, we do not accept making appointments or reservation changes by e-mail.

You can contact us in advance, but you should make an appointment after confirmation of your flying date .

Examination day: Tuesday and Thursday (9:00-12:00 & 13:30-15:00)

If you need the RT-PCR test on another day because of flight schedule, please tell us when you need an appointment.

When you come for an examination, please bring your passport or it’s copy.

Process of examinations on the visiting day

1. To perform the procedure for the first visit at central reception desk

2. To check in at the reception desk 102 (the Department of Infectious Diseases (Travel clinic) counter).

3. To check fever and take interview and physical examination by an attending physician

4. To take nasopharyngeal swab test for RT-PCR

5. After confirmation of how to receive the certificate, proceed for payment.

The above order may change depending on the situation.

◆Time Required for Health Certificate Completion

You can receive the health certificate including the result of RT-PCR test after 8:30 a.m. the next day. You don’t need the process of return visit. Please come directly to the reception desk 102 and show your patient ID card. You can receive the health certificate on only weekdays.

 (Someone with letter of attorney can receive the health certificate instead of you. If you want, please tell us when you need an appointment.)

◆Health Certificate Fee

The fee for a health certificate depends on the content of your examination. As examples, costs in weekdays for the following documents using our hospital forms are as below:

The fee for the health certificate including the result of RT-PCR test is about 20,000 yen (tax included, by patient’s own expense, i.e., not covered by Japanese health insurance).
